Surgical Treatment Options for Sleep Apnea
When CPAP and Oral Appliances Are Not Enough
At Innovative Dental Sleep of Reno, we help patients across northern Nevada find real, lasting solutions for obstructive sleep apnea. For many, that means oral appliance therapy or lifestyle changes. But for others, especially those with more severe cases or anatomical challenges, surgery may be the most effective option.
For qualifying patients, there are four surgical options currently available, which include the hypoglossal nerve stimulator (INSPIRE), Maxillomandibular Advancement (MMA), Tracheotomy, and Uvulopalatopharyngealplasty (UPPP). These procedures are typically considered when traditional therapies have failed or are not appropriate.
Patients from Reno, Sparks, Sun Valley, Spanish Springs, and Incline Village trust our team to help guide them through every step of the sleep apnea treatment journey, including surgical care when needed.
Hypoglossal Nerve Stimulation (INSPIRE)

A small implanted device that helps keep the airway open
INSPIRE is an FDA approved therapy that uses a small device implanted under the skin to stimulate the hypoglossal nerve, which controls tongue movement. It gently activates the airway muscles during sleep to prevent collapse and improve airflow.
Why patients consider INSPIRE
• A mask-free alternative to CPAP
• Controlled by a handheld remote
• Turned on at night and off in the morning
• Often used when CPAP is not tolerated or not effective
INSPIRE is a newer option that combines advanced technology with a less invasive approach to improving sleep apnea.
Maxillomandibular Advancement (MMA)

Repositioning of the jaws to open the airway
Maxillomandibular Advancement is a procedure that moves the upper and lower jaws forward. This shift increases the space in the airway behind the tongue and soft palate, where obstructions often occur during sleep. By changing the shape and support of the airway, MMA can lead to dramatic improvements in breathing, sleep quality, and overall health.
Why patients consider MMA
• It provides a permanent structural solution
• It is often more effective than CPAP or oral devices in certain cases
• It helps eliminate airway collapse without relying on daily equipment
• It is commonly recommended for patients with moderate to severe OSA who have not responded to other treatments
MMA is a well-studied and highly effective procedure that can significantly reduce or eliminate sleep apnea symptoms.
Tracheotomy

Creating a new path for breathing during sleep
A tracheotomy involves placing a small opening in the neck that connects directly to the windpipe. This allows air to bypass the blocked upper airway entirely. While it may sound extreme, this option can be life saving for patients with very severe or complex sleep apnea, especially when other treatments have not worked.
When a tracheotomy may be considered
• For patients with life threatening sleep apnea
• When other surgical options are not possible or have failed
• As a last resort for maintaining an open airway during sleep
This procedure is rarely the first choice, but in select cases, it offers a reliable way to restore breathing and prevent serious complications.
Uvulopalatopharyngoplasty (UPPP)

Surgical removal of excess tissue in the throat
UPPP is a procedure that removes the uvula and part of the soft palate to reduce tissue obstruction in the upper airway. It is most often used to treat snoring or mild to moderate sleep apnea.
When UPPP may be considered
• For patients with obstruction in the soft palate area
• When less invasive treatments have failed
• Sometimes used alongside other surgical approaches
While not appropriate for every case, UPPP can be effective when the airway blockage is isolated to the upper portion of the throat.
